We had first consulted Out Patient and underwent few investigations and later after couple of days admitted as In Patient and discharged yesterday. Patient is a state pensioner dependent (wife) and do we need to mention both (OP and IP) investigation reports and medicines purchased under the essentiality certificate to be procured as a part of medical reimbursement.. please clarify..
